After scheduling your appointment, you will receive an email to complete a consent form. This must be filled-out and submitted before your appointment!
This is a telephone appointment. The nurse will call you at the scheduled time.
If you would like us to process your registration application/renewal with the state online, we will also need pictures of a valid form of ID and a secondary proof-of-residency document. (car registration, utility bill, etc.) If you have already processed, or intend to process your own registration application, you can disregard this.
If we do not receive those documents, we will be unable to process the online registration on your behalf. We do our best to follow up with patients when we have not received them, however, patients are ultimately responsible for sending them to us in a timely manner.
Documents can be uploaded by pressing the button below, or via the "Upload Documents" tab on our website.
Please note that uploading documents for registration is not the same thing as the consent form.
Please do not upload documents for registration until after you have scheduled your appointment above.
